This sounds just like this issue I reported a while back with the Rep 1 Dual. I've not had time to try 2.1.0 to see if it is present there.

2.1.0 crashes when a tablet is used on Mac OS X

I reported this as a bug to support. I have a Wacom tablet. If I run the app using that as my pointing device, it crashes immediately. If I use a mouse, I'm fine. Left and right are perfectly positioned if I slice with Rep G. Is that still possible if the offsets are not set correctly?

Raw gcode. I could look at the binary but haven't done that.

Nope. Rep G does not apply offsets. I diffed the gcode because I wasn't sure how it wad handled. It's hard to mix the code up too because the creator is in the code. I don't enter toolhead offsets into S3D, but it does know that I have a Rep 1 Dual and am using Sailfish, so it has the stock offset (...

More info. The offset is 18.6mm, and the same model (20mm test cube) prints centrally on both extruders with Replicator G (latest Sailfish version), so it's not the Replicator. Looking at Rep G geode, I see that it only swaps the tool heads to switch extruders (TO->T1 throughout). Simplify 3D swaps ...
