Ended up talking to S3D tech support. They made a few suggestions, but the first one I tried was reducing the layer height from 0.2 to 0.1mm and that completely solved the problem

If you look at the photo carefully, it's doing straight lines between points inside the hole in an octagon like shape rather then the curve necessary for the thread.
