Can you post your factory file ? 13mm offset seems odd.. Are the toolheads interchanged ? Is Left extruder the Primary tool (T0) ?

If you change your hotend, then it is likely to cause issues with the preset Z height in the firmware and build height in the software, you have to update that

As long as the e3d is assembled correctly, it will not cause any issues. Is it the genuine or a cheap clone ? Is the hotend fan running continuously ? Is the direction of the fan (physical) set correctly ?

This issue looks very similar to the nozzle scoring on top surface quality issue. Also, when a model loses contact with or passes through the floor plane, is there a way to level the lowest point with the floor plane automatically without eyeballing, or without changing the X and Y coordinates for all models the way "Center and Arrange" does? Have you tried playing with the extra restart distance ? From the looks of it, it is overextruding slightly at the beginning of the print, could also be ooze. I would definitely tune the retraction, especially the length and the extra restart.

hmmm.. Ooze shield and Prime pillar both indirectly do toolchange priming. Try increasing the ooze shield outlines. If it is not the filament that is causing the problem, it could be some sort of contamination in the nozzle, try cleaning the nozzle/coldpull or replacing it. The toolchange retraction...
