Hot end won't reach set point

This started right after update. Using single nozzle with set point at 200c, I have a hot bed but it's not being used. Using USB cable. Start a print and it slowly climbs to around ~193 and stalls, printer times out. So I reset the set point to 193 and it climbs to ~186, stalls and times out. I go to the printer control panel and it ramps up to set point no problems. I'm stumped, any ideas?

If you can manually send a M190 S200 and it never reaches that temperature, then it definitely doesn't have anything to do with the software. My guess would be probably your electronics or the firmware tuning.

Is there a PLA cooling fan that is coming on early? Any insulation on the hot end?

I do have a cooling fan but it's not coming on. The small fan connected to the extruder stepper motor comes on but that shouldn't affect the hot end should it? What stumps me is the fact that everything works fine from the printer control panel but not from Simplify3d.

Again, I doubt it has anything to do with the software. Open up any program you want and send M190 S200. My guess is the result would be the exact same since the firmware is the one that controls the preheating process.

Along with what Carl has suggested I might try to reset S3D back to its defaults and reload the printer profile. this can help sometimes. Go to the help menu and you will see the reset option.
