missed layer(s)

OK, I've been battling this one for quite some time. Whenever I have a hole in the side, i have an imperfection on the entire layer at the center of the hole (sometimes effects a few layers) It looks kinda like under extrusion. it doesn't seem to matter which slicer(slic3r, cura, or S3D). Heated bed doesn't matter(tried no heat, and 50-70). print temp 180-215. Hatchbox Red PLA. Think I got it with Overture PETG also, but have long since discarded those parts.

these parts are sliced with S3D, .2 layer, 20% infill, 60 bed, 200 extruder, 3600mm/in, 40% base layer speed at .3
printer is a custom built CoreXY
bad layer 1, note the 2 bolt holes, that correspond to the missed layers.
bad layer 3, its like it missed an few layers, the part split. There was another part printed with this one, which also split in half, (but didn't have horizontal holes)

including supports does seam to lessen the effect. The pic that has 3 parts top left part has supports in the bolt holes

Yes, I'm aware of the overly squashed(elephant foot) base layer, done on purpose, because I'm have a heck of a sticking problem, and warping when the part reaches about 1" high, no matter what surface, glass, powder coated spring steel, with an combination of glue, tape, and hairspray, temp, speed etc
Can anybody give me some idea as to what is causing this, and/or how to prevent it.
