Yeah the Factory File still shows no Y offset coming from Simplify3D. The gcode the software is creating is correct but for some reason your printer is offsetting the Y.
It almost looks like this may actually be some severe layer shifting occurring and not an 'offset' so to speak. Could you try the same print but slow the Default Print Speed from 3600 to 2400mm/min or so?
