Hi

Is there a setting to make the top layer smooth and/or have the layer start from one side and end at the other to get a better top layer?

The biggest thing that has helped me is slowing down the speed at which it prints the top layers. Look in the Other tab, setting called "Solid infill underspeed". I use a value around 50-60% and it helps a lot.

My rectilinear infill already starts at one side and works its way across to the other side, so I'm not sure what you're talking about there...

I've only seen this happen when there are internal features on the top layer.

Yes there is multiple cutouts on the top layer that it fills in different areas at different times and just does not start from one end to the other.
