Bed Leveling and Z0

I understand how to level the build platform and have done it successfully many times.
Here is my question: Once bed is leveled, say with a slip of paper, do you then leave that as Z 0.0? Or do you create an offset to compensate for the .003" slip of paper. If so then your print will start at .003" above bed.

The bed leveling wizard in S3D actually allows you to input the size of the object you use between the nozzle in the bed (i.e. input the thickness of the paper you used). Then it automatically adjusts for that thickness so that you don't need to do it yourself.

If you run a print and find that you still need some additional adjustment, just use the Z-axis global gcode offset (on the G-Code tab). For example, entering 0.1mm would move the nozzle 0.1mm further away from the bed.
